ConSASS is the Construction Safety Audit Scoring System, a standardized audit checklist and scoring system used to assess the effectiveness and maturity of a construction worksite’s Safety and Health Management System (SHMS).
Construction worksites with a contract sum of S$30 million or more are required to undergo mandatory ConSASS audits.
ConSASS comprises an audit checklist, interview sheets, and a scorecard used to assess the effectiveness and maturity level of a company’s Safety and Health Management System (SHMS).
Only approved WSH auditors from MOM accredited auditing organizations can submit ConSASS score card using their SingPass.
You must attach the completed ConSASS checklist, scorecard, and final audit report during submission.
Supported formats include Microsoft Word, Microsoft Excel, PDF, txt, JPEG, and TIF, with individual files limited to 3 MB or lower.
Approved WSH auditors can submit and amend ConSASS reports.Company representative of auditing organisation/construction company can enquire, submit and amend submission status via the MOM WSH e-Services.
MOM may contact auditors via SMS or email if amendments are required. No further contact will be made if the submission is accepted.
If it’s returned by MOM, auditor will be required to retrieve the draft from "Returned Reports," make the required changes, and resubmit.

The ConSASS audit checklist is a comprehensive tool used by MOM-approved auditors to systematically assess a construction worksite’s Safety & Health Management System (SHMS). It consists of 204 structured questions grouped into Bands I–III, covering key PDCA cycle stages:
Each question specifies the required audit method - Document Review, Interviews, or Physical Inspection - with auditors marking responses in “Yes/No/N/A” and providing comments. A minimum of 70% compliance in Bands I & II is required to progress to Band III.
The Questionnaire for Interview of Personnel is intended to guide auditors in conducting informal conversations with various site personnel - such as managers, supervisors, workers, and contract staff, during the audit. The questions are derived from the main checklist and may be applicable only to certain groups (e.g., management-level staff). For audit items assessed using the Interview of Personnel (IP) method, at least 70% of interviewees must provide a positive response for credit to be awarded.
The ConSASS scorecard is a summary tool that tabulates audit results across the 20 SHMS elements evaluated. Auditors record the highest Band (I–III) achieved per element, input percentage scores, and fill in the necessary particulars including the company being audited, dates of audit, and the name of the lead auditor in the audit team with his/her signature.
The completed scorecard visually represents the maturity profile of the site's SHMS, helping management identify areas for improvement.
